Home Health Program (HT2)

HealthCare Talent is one of Southern California’s largest third party providers of non-medical, in-home care workers for the elderly. We recruit, hire, and manage skilled home care workers for placement in the homes of our clients’ patients.

HealthCare Talent is responsible for:

  • Recruiting: HealthCare Talent provides our clients with qualified, well-trained, and experienced in-home care workers (caregivers)
  • Compliance oversight: All caregiver files are compliant per Centers for Medicare and Medicaid Services (CMS) regulations
  • Care plan review: On-going review of care plans with case management and social workers
  • Superior customer service: Assess and respond to all client inquiries, fill all requests for service and build and maintain strong client relationships
  • Quality: Improve home health care quality and patient safety through interaction with families and participants to ensure a high level of satisfaction.
  • Training: On-going education to our caregivers on various subjects including, but not limited to: Infection Control, Alzheimer’s/Dementia, Fall Prevention, In-home safety and Managing Caregiver Stress
  • Cost savings: Strategically partner with our clients to mitigate costs related to home care services
  • Emergency disaster planning

Our caregivers come from diverse backgrounds. Many are fluent in various languages.

Some of those languages are:

  • Spanish
  • Mandarin
  • Cantonese
  • Tagalog

 

PACE Programs

Program of All Inclusive Care for the Elderly

HealthCare Talent has developed an all-inclusive national managed service program to support healthcare organizations that have received Medicaid funding for PACE programs in their geographic area. The Program of All Inclusive Care for the Elderly (PACE) creatively coordinates the care of each participant enrolled in the program based on his or her individual needs with the goal of enabling older individuals to remain living in their community instead of going to a skilled nursing home or other long term care facility.

To qualify to enroll in PACE the participant must be:

  • 55 years of age or older;
  • Meets the requirement for skilled nursing home care as determined by the PACE organization’s interdisciplinary team assessment and certified by the California Department of Healthcare Services
  • Can live in the community without jeopardizing his or her health or safety.

With PACE, the focus is on the quality of patient care —we have a team of health care professionals working to ensure our participants receive the coordinated care they need.
